Tom Stall (Viggo Mortensen) runs a diner, along with his wife Edie (Maria Bello), on the outskirts of a small Indiana town. One day two convicts on the run, step into the diner and hurt one of the customers, and it seems that killing others will present no problem to them. Tom steps up and wrestles a gun from one of them and takes them out before they can hurt anyone else, and as such he is hailed as a hero. But Tom isn’t eager on this newfound attention, and it seems he may have attracted some wrong attention too…

On another normal-seeming day, a man with severe facial scarring steps into the diner and sits at the counter directly facing Tom, who he starts referring to as Joey and begins asking him about the old days back in Philly. While a confused Tom is sure that this is a case of mistaken identity, the man who goes by the name Carl Fogarty (Ed Harris) seems to have a very dark and violent past and is here to settle some unfinished business…
